What we collect
The assessment can be completed without submitting personal details. If you choose to share your result, we collect your full name, clinic or organisation, work email address, optional telephone number, clinic or organisation type, approximate team size, main 12-month growth objective, contact preference, 12 assessment responses, four pillar scores, overall readiness score, readiness stage and identified priority. We also record the date and time of submission.
Please do not include patient information or other sensitive personal information in this assessment.
Why we use it
With your consent, we store and review the assessment result and details you submit. We will contact you about a complimentary strategic conversation only if you select the separate contact-request checkbox or clearly ask us to respond by another method. We do not use assessment submissions to add people to unrelated marketing campaigns.
We may rely on our legitimate interests to operate and secure the service and maintain appropriate business records, subject to balancing those interests against your rights. Where the law requires us to keep or disclose information, we rely on the relevant legal obligation.
Who can see your information and the services we use
Access is limited to Vanessa Barnes and any other Insignia personnel formally approved to administer the assessment. The current private response page requires ChatGPT sign-in and is restricted to the approved site-owner account. We do not sell your information.
The assessment is hosted using OpenAI/ChatGPT Sites, and submitted responses are stored in the Site’s database service. OpenAI’s precise contractual role, relevant subprocessors, data locations, retention arrangements, data-processing terms and UK international-transfer safeguards for this Site are still being verified by Insignia and must be recorded before a wider launch.
Assessment responses are not currently transferred to RecruitCRM, and the Site does not currently send automated email notifications. If either process is introduced, this notice will be updated to identify RecruitCRM and the relevant email provider, explain the purpose and lawful basis, and give verified information about any international transfer before that processing begins.
International transfers
Insignia has not yet completed its provider-specific international-transfer assessment for OpenAI/ChatGPT Sites. We will not describe a safeguard that has not been verified. This assessment is therefore intended only for a controlled pilot until the relevant contractual terms, processing locations and any required UK transfer mechanism have been confirmed and documented.
How long we keep it
Assessment leads that do not progress will be deleted or anonymised 12 months after the last meaningful interaction. Vanessa Barnes will review the Site responses at least quarterly and apply this rule using the secure deletion control. Active opportunities and client records will follow the relevant approved business-record schedule. You may request earlier deletion where the law permits.
Your choices and rights
You may withdraw your consent or ask us to delete your assessment submission at any time. Depending on the circumstances, you may also have rights to access, correct, erase or restrict your information, receive a copy of it, and object to certain uses.
To exercise a right, email VBarnes@insignia-group.co.uk. We may need to verify your identity before acting on a request. Withdrawing consent does not affect processing that took place before withdrawal.
Assessment results
The readiness result is generated automatically from 12 responses across four pillars: commercial growth, workforce capacity, governance and CQC readiness, and leadership bandwidth. Each pillar score reflects its three responses, and the overall score combines the four pillar scores. Where scores are tied, the result presents the tied pillars together rather than selecting an arbitrary strongest area or constraint.
The result is a directional business-development tool and conversation starter. It is not clinical, legal, regulatory or CQC advice and does not make a legal or similarly significant decision about you or your organisation.
Cookies and security
No advertising, analytics or marketing-tracking technology has been added to the Site’s application code. The hosting platform may use technologies needed to operate and secure the service; its precise cookies and similar technologies are being verified. If non-essential technology is introduced or identified, it must not operate until valid consent has been obtained and suitable cookie information and controls are provided.
We use reasonable technical and organisational measures to protect submitted information, although no online service can guarantee absolute security.

Changes to this notice
We may update this notice when the assessment or our legal obligations change. The latest version will always be available on this page and will show its revision date.
